This
agreement is between The Consultant and The Client for the provision of consultation services by The Consultant
with The Client.
Either party of the agreement may
terminate services at any time while the agreement is in effect, on provision of written notice prior to the termination.
Areas in which The Consultant is competent to provide consulting
service include: motivation, goal setting, imagery, self-talk, emotional and attentional control, confidence, competition
routines, communication, leadership, motivational climate, team building, slump busting, and the psychology of injury recovery.
Although every attempt will be made to satisfy The
Client’s needs, change is not guaranteed. The Client is solely responsible for change.
Required services outside of the above areas of competence will facilitated through referrals by The Consultant in
consultation with The Client. Costs associated with the additional treatment resources will be paid by The Client to other
professionals. Such services may include treatment for substance abuse, eating disorders, or other forms of psychotherapy.
Issues which require police notification (i.e., Duty to Warn) will be addressed without consent of The Client.
Fees for the provision of services by The Consultant will normally be
in the amount of $70 per hour, $260 per half day (4 hours), $500 per day (8 hours), or $2,000 per week (5 days) paid to The
Consultant by cash or check.
Expenses also will be charged. Full costs of reasonable accommodation and travel will be charged to The Client;
the maximum daily meal allowance charged to The Client will be $60.
At the beginning of the term of this contract, The Consultant will hold a free introductory session with
The Client in order to outline expectations for the provision of services and to answer questions from The Client.
The nature and content of all services provided by The Consultant are privileged
and will not be disclosed by The Consultant to anyone without oral or written consent by The Client. However, at the
end of the service period The Client will be obliged to complete a questionnaire assessing the experience working with The
Consultant, including providing a testimonial statement which may be used to promote The Consultant's service (e.g., posted
on this website).
Any modifications to this agreement must be made by
mutual consent of the parties.
